Nurse Case Manager/Delegating Nurse

JOB DESCRIPTION
 
POSITION TITLE:                       RN Nurse Case Manager/Delegating Nurse

ORGANIZATIONAL RELATIONSHIP:
 
            REPORTS TO:              Nurse Managers, Director of Clinical Operations and President/CEO
            STATUS:                       Salaried/Exempt or Hourly/Non-exempt
 
OVERVIEW:                  The Nurse Case Manager/Delegating Nurse is responsible for the ongoing care of the clients that are assigned to him/her according to the guidelines set forth by the Developmental Disabilities Administration, Maryland Board of Nursing, and outlined in the DHCA Medication and Nursing Related Policies and Procedures manual. This responsibility also includes the delegation of and oversight of approved functions to agency staff that work at the client’s residence.
 
QUALIFICATIONS:
 

  1. AS, AA, or BS Degree in nursing is preferred.
  2. Current/valid Maryland RN license or current/valid multistate compact license is required.
  3. Minimum of one (1) year experience as a nurse is preferred.
  4. Experience in the developmental disabilities environment is highly desired.
  5. Excellent verbal, written, and interpersonal communication skills.
  6. Must have current valid state-issued driver's license and own reliable transportation.
  7. Must have a working “Smart” cell phone capable of making calls, receiving calls, receiving emails, and sending emails.

 
PHYSICAL REQUIREMENTS:

  1. Must be able to fluently speak and understand spoken English.
  2. Must be able to fluently write and read English.
  3. Must be able to answer phones.
  4. Must be able to lift a minimum of 30 pounds up to 4 times during a scheduled work day.
  5. Must be able to sit and look at a computer for 6 hours during a scheduled work day.
  6. Must be able to type on a computer (both laptop and desktop) keyboard and utilize a computer mouse for 6 hours during a scheduled work day.
  7. Must be able to adapt to technology.
  8. Must be able to walk up and down a minimum of 20 stairs.
  9. Must be able to stand for a minimum of 90 minutes.
  10. Must be able to bend, kneel, and squat throughout the day.

 

RESPONSIBILITIES
 

  1. Serve as the Delegating Nurse/Case Manager for each client assigned.
    1. When the client is first assigned, visit the client in the environment in which delegation occurs, conduct and complete an initial clinical assessment and develop the nursing plan of care.
    2. Complete Choking Risk Screening Tool and Feeding Protocol as necessary.
    3. Every 45 days, visit clients in the environment in which delegation occurs to assess their physical condition, review medications, documentation, prescriber orders, and review medical appointment/screening exams/immunizations/lab work as needed, and provide necessary care.
    4. Visit clients who are not chronic, stable, routine, uncomplicated, and predictable minimally every 2 weeks in the environment in which delegation occurs.
    5. Conduct Interim visits for clients in their residence and/or day/vocational/supported employment setting as needed for occurrences such as post-ER visits, after hospital discharge, dressing changes, falls, injuries, etc. Visit must be completed within 48 hours of incident.
    6. Delegate administration of medication and other delegatable tasks to certified Medication Technicians.
    7. Train and ensure staff’s competency in nursing care plans, delegated nursing tasks, feeding protocols (if applicable), and any other necessary tasks, protocols, or forms.
    8. Monitor and report medication errors made by residence staff.
  2. When in the office, field calls from staff at residences regarding clients’ care, review and file medical records, assess and/or provide care to clients, and perform any other duties as assigned.
  3. May be assigned on-call responsibility from the time the office closes until it opens the next morning, one weekend every 10-20 weeks. Must be available by cell phone during this time.
  4. Conduct medication administration practical exams for Medication Technicians.
  5. May be assigned to teach Medication Technician courses, CPR, OSHA training, First Aid, Seizure Disorder, Nutrition, and Skills classes.
  6. Maintain RN License, CPR certification, Malpractice Insurance, and Auto Insurance.
  7. Obtain RN CM/DN certification, Online HRST Rater Training, and Online HRST Reviewer Training.
  8. Complete, review, and update Online HRSTs as needed.
  9. Attend two mandatory Developmental Disabilities Administration (DDA) nurse quarterly meetings per year.
  10. Other duties as assigned.
